Elite Family Systems provides guidance and support in transitioning children from our residential program to family settings without major disruption in their treatment plans. We help them understand the transition process. Then we support them through a continuum of care. On top of these, we do the critical transition planning with our residents’ new guardians to solidify the support we can give them.
