Targeted case management is especially aimed at individuals with cases such as our residents. Our targeted case managers ensure that our residents and high-risk individuals get and continue to receive the support they need to access health care, education, or other services. They conduct:
- community-based assessment and planning with everyone that should be involved in a recipient’s care
- service referrals
- care/service coordination
- progress monitoring
